Community wellbeing officer to work in Hospital Social Care Team full time 37 hours weekly. The team is a dynamic, multidisciplinary group committed to supporting safe and timely hospital discharges. We work closely with health professionals, patients, and families to ensure that care planning is person-centred and promotes independence wherever possible.
The Hospital Social Care Team supports safe and timely hospital discharges by coordinating person-centred care plans that promote independence and recovery. Our work reduces delayed transfers of care, improves patient outcomes, and strengthens collaboration across health and social care systems—benefiting both the borough and the wider authority by easing system pressures and ensuring efficient use of resources.
The core responsibility of the Community wellbeing officer is to carry out timely, strengths-based assessments and coordinate safe discharge plans for patients leaving hospital care. They will be expected to reduce delayed transfers of care, ensure legal and safeguarding duties are met, and contribute to improved outcomes through collaborative working with health and care partners.
The successful candidate
The successful candidate will also be responsible for completing mental capacity assessments, supporting with safeguarding enquiries, and contributing to multi-disciplinary team discussions. They will need to demonstrate strong communication, decision-making, and organisational skills, as outlined in the person specification, to ensure effective care planning and advocacy for vulnerable individuals.
