Role: Triage Nurse

Location: Bury & Rochdale Primary Care Sites
Contract Type: Full-time / Part-time / Shift-based
Salary: £26 - £30 LTD

The Clinical Navigator is the first point of contact for patients accessing services to ensure patients receive timely, appropriate, and compassionate support by assessing needs, managing appointments, and directing individuals to the most suitable healthcare professional or community service.

Working within a multidisciplinary primary and urgent care environment, the Clinical / Care Navigator supports effective patient flow, promotes safe access to healthcare services, and helps patients navigate NHS systems and local care pathways.

Key Responsibilities

  • Act as the first point of contact for patients via telephone, online systems, and face-to-face interactions.
  • Manage and triage patient requests using agreed care navigation protocols.
  • Direct patients to the most appropriate clinician or service, including GPs, nurses, pharmacists, social prescribers, or external community services.
  • Support vulnerable or distressed patients with empathy, professionalism, and reassurance.

Essential Criteria

  • Previous experience in primary care, urgent care, NHS services, or a patient-facing healthcare environment.
  • Excellent communication and interpersonal skills.
  • Strong organisational and multitasking abilities.
  • Understanding of patient confidentiality and safeguarding responsibilities.
  • Good IT and administrative skills, including experience using healthcare systems.
  • Ability to work effectively as part of a multidisciplinary team.
