Key Responsibilities:

• Conduct assessments, screenings, and chronic disease management.
• Deliver immunisations, wound care, and health education.
• Collaborate with the GPs and clinical team to develop patient treatment plans.
• Contribute to a culture of proactive, preventative healthcare.

About You:

• Registered with the NMC, experience in a similar role is desirable but not essential.
• Skilled in-patient consultations, with a compassionate approach.
• Strong team player who is adaptable and proactive.
• Qualifications in chronic disease management are desireable.
