Community Wellbeing Coordinator - fixed term/secondment opportunity until 31 March 2028 due to funding.
Salary: Band: 7 - £32,061 to £36,363 pa, (£25,995 - £29,483 for 30 hours)
30 hours per week - to be discussed at interview.
Roles and locations: Three roles available with a focus in the below localities:
Community Wellbeing Coordinator (30 hours) - Portsea and Somerstown

The Wellbeing Service supports individuals living in Portsmouth to modify behaviours, improve their health and life expectancy as well as reduce health inequalities. The service provides support to Portsmouth residents in a range of venues including community centres, GP practices and hospital settings. In addition, the service provides telephone/virtual support. Some of the work undertaken will be targeting Portsmouth's most deprived communities who unfairly suffer the greatest health inequalities.
This is an exciting opportunity to lead, coordinate and support a newly established team of Community Wellbeing Workers, people recruited from their communities to assist in improving the health and wellbeing of residents across those communities. The role combines workforce development, service delivery and health improvement, with responsibility for mentoring and supervising staff, assessing performance, sharing best practice and contributing to the development of new ways of working.
Working as part of a rota, including 1 evening shift per week, you will engage directly with clients in a variety of community settings, carrying out assessments and identifying the support they need to make positive lifestyle changes. You will provide personalised support around smoking cessation, physical activity, weight management and alcohol reduction, while also addressing wider factors that impact health and wellbeing such as housing, debt, social isolation and domestic abuse. Through a combination of brief interventions, behaviour change support and long-term engagement, you will support individuals with the tools to improve their health and achieve sustainable outcomes.
The role also involves maintaining accurate client records, supporting referral and triage processes, delivering health improvement initiatives and representing the service at community events. You will act as a key point of contact for colleagues and partner organisations, creating and delivering training programmes and supporting the development of both staff and stakeholders. Working closely with health and care partners across the city, you will contribute to collaborative approaches that improve population health and support local responses to public health priorities and emergencies.
Who is the person summary:
A strong understanding of public health, health inequalities, wider determinants of health and local community needs is essential, alongside the ability to engage and support individuals through personalised health improvement plans. You will maintaining accurate client records using Microsoft Office applications and caseload management systems.
You will have an excellent level of knowledge and technical skills when delivering behaviour change interventions relating to physical activity, weight management, smoking cessation, alcohol reduction and emotional wellbeing. You will have experience in planning, delivering and evaluating targeted public health projects and creating and delivering training programmes.
In addition, you will have excellent interpersonal skills as you will be working collaboratively with healthcare professionals, community organisations and voluntary sector partners. Experience supporting, supervising or developing colleagues, leading community-based clinics and working as part of a multidisciplinary team is required.
An effective communicator with excellent analytical and organisational skills, you will be capable of managing a varied workload, interpreting data, producing reports and presentations. Candidates must demonstrate integrity and compassion, self-motivation, a commitment to safeguarding, equality and continuous professional development. You will have the flexibility to work across Portsmouth (PO1-PO6).
