Full job description
Service Overview
As the successful postholder, you will be an experienced healthcare practitioner with a background in acute and urgent care. Ideally, you will have completed an MSc in Advanced Practice and be an independent prescriber. You will be able to demonstrate a clinical portfolio and relevant competencies in line with the HEE National Framework for Advanced Clinical Practice. As an Advanced Clinical Practitioner, you will work across the four pillars of advanced practice: clinical practice, leadership, research and education.
Working within our remote clinical services, you will practise as an Advanced Paramedic Practitioner, supporting the Acute Winter Hub.
In this role, you will autonomously assess and treat patients presenting with both acute and chronic conditions, including differentiated and undifferentiated presentations. You will determine clinical diagnoses, develop treatment plans and arrange appropriate discharge or referral pathways while maintaining accurate patient records and ensuring patient safety.
You will act as a source of clinical expertise for less experienced colleagues, helping to develop skills and knowledge across the team while working collaboratively within a multidisciplinary environment.
You will play a key role in ensuring a high-quality, patient-centred service is delivered across all remote clinic settings.

We have a number of remote shifts available during service hours 09:00 - 17:00 Monday to Friday. Shifts are available on a remote basis, though regular travel to the office will be required for team meetings.
Primary Duties and Areas of Responsibility
Clinical Responsibilities
As the successful postholder, you will:
- Independently undertake specialised clinical assessments, diagnose, treat and discharge, or appropriately refer, patients presenting with a wide range of acute and chronic illnesses.
- Ensure patients are fully involved in the development and implementation of their treatment plans.
- Coordinate and liaise effectively with multidisciplinary teams across primary and secondary care, statutory services and voluntary organisations to provide seamless patient care and prevent hospital admissions where urgent care is required.
- Seek guidance from Clinical Leads or supervising GPs when a second opinion is required.
- Adapt your communication style to empower patients, promote self-management and support health education.
- Attend clinical case conferences, reviews and meetings as required.
- Provide clinical expertise and support to less experienced members of the team.
- Delegate duties and tasks appropriately to other team members.
- Provide specialist advice relating to minor illness and/or injury to local healthcare teams, patients, relatives and carers.
- Identify actual or potential clinical risks and take appropriate action, following the relevant reporting procedures where necessary.
Leadership and Service Delivery Responsibilities
As the successful postholder, you will:
- Ensure patient records and documentation are maintained in line with professional standards, organisational policies and the electronic patient record system, including:
- Maintaining full and accurate records of assessments, investigations, procedures, treatments and aftercare.
- Supporting colleagues to maintain high-quality record keeping.
- Working closely with administration and operational teams to ensure all relevant patient information is appropriately documented and filed.
- Contributing to the accurate read-coding of patient data.
- Work in accordance with local and national guidelines, policies and protocols.
- Participate in audits and use findings, current research and government guidance to recommend and implement improvements to service delivery and clinical practice.
- Support the development and maintenance of effective communication networks across all disciplines to ensure continuity of care.
- Collect and provide service data and performance information as required by the Service Manager.
- Contribute to the development of systems for gathering and acting upon patient feedback.
- Ensure Service Managers and Clinical Leads are kept informed of service issues, concerns and potential risks.
- Work collaboratively with colleagues to deliver and promote the objectives of the service.
- Support the Service Manager and Clinical Lead in meeting clinical governance requirements, including the investigation of incidents, serious incidents and complaints, and promoting learning from outcomes.
- Provide clinical supervision and mentorship to junior practitioners where appropriately trained.
- Maintain up-to-date knowledge regarding all equipment used within the service and report any faults or technical issues promptly.
- Participate in clinical team meetings and contribute to discussions regarding day-to-day operational matters.
