Founding Medical Director (GP) for a new nurse-led iron infusion clinic in central London. 3–5 hours a month, almost entirely remote. £1,200–£1,500 a month plus £15 per patient treated and 3–5% equity. Protocol development from November 2026; first patients early 2027.

About IronUp

IronUp is a clinic treating confirmed iron deficiency in otherwise healthy women of reproductive age who have failed or cannot tolerate oral iron. The scope is deliberately narrow and will stay that way: anyone outside it, including men, postmenopausal women, unexplained anaemia and patients with significant comorbidity, is declined and directed to their GP or a consultant-led service.

The gap we are addressing is well known to anyone in primary care. These patients are rarely offered IV iron on the NHS, and the private alternative in London is consultant-led and priced at £670–£920. In Australia the same treatment is routine general practice at a fraction of the cost. We are bringing that model to London: a registered nurse delivers treatment from CQC-registered premises, a GP reviews every result before a patient is booked, and the price is fixed and published.

The founder is a software engineer, not a clinician, and started the company after a family member went two years without treatment here and was treated within a week in Australia.

A sessional GP is in place for day-to-day results review and prescribing, a GP clinical adviser is contributing to the protocol, and a registered nurse is being recruited as clinical lead.

The role

The Medical Director is the senior clinician in the business and owns its clinical governance. Specifically:

- Finalise and sign off the clinical protocol before launch (eligibility and exclusion criteria, dosing, administration, management of hypersensitivity reactions, follow-up), and review it annually

- Approve the prescribing framework and provide clinical supervision to the sessional GP

- Monthly review of the infusion log, incident and reaction log, and outcomes audit

- Telephone availability for escalation on clinic days (two days a week at launch)

- Quarterly governance meeting in person with the founder and clinical lead nurse

- Clinical point of contact for the host premises, the indemnity provider and the CQC

What you would shape

- The protocol itself, built on the BSH guideline, NICE CKS and the product SmPC, drafted with our GP adviser and finished under your authority

- The outcomes audit. Every patient is logged from the first: haemoglobin and ferritin before and after, adverse events, retreatment. A nurse-led community IV iron pathway with complete outcome data is publishable, with the Medical Director as first author

- The clinical standard for subsequent sites. The plan is to open further clinics in London and then regionally; the Medical Director of the first becomes Clinical Director of the group

Terms

- Retainer of £1,200–£1,500 a month for 3–5 hours of work, plus £15 per patient treated. First-year planning assumption is 20–40 patients a month

- One-off fee of £1,500–£2,000 for pre-launch protocol development, payable regardless of launch date

- Equity of 3% for the role as described, rising to 5% for a founding clinical partner who takes on more (nurse training, public clinical lead, expansion), vesting over four years with a one-year cliff

- Role-specific indemnity arranged and paid by IronUp

- A written scope of responsibility: governance and protocol, not the administration of individual treatments

- Self-employed, invoiced monthly. Location flexible; London or the South East preferred for the quarterly meeting

Requirements

- Full GMC registration with a licence to practise and inclusion on the GP Register

- Three or more years post-CCT; five preferred

- Confident management of iron deficiency in primary care. Prior experience with IV iron is welcome but not required; a willingness to master the evidence and own the protocol is

- Previous clinical lead, medical director or governance experience is an advantage

- Willingness to act as practice assessor for the clinical lead nurse's V300 independent prescribing course would be a significant advantage

Governance and safety

Treatment takes place on CQC-registered premises with resuscitation equipment and anaphylaxis management on site. Every patient has a documented screening for hypersensitivity history, asthma and atopy, first-trimester pregnancy, active infection and iron overload, and is observed for 30 minutes after infusion. Dosing is on ferritin, never on haemoglobin alone. Patients outside scope are referred, not treated. We expect you to test all of this before you put your name to it.
