An exciting opportunity has opened up in our Team as a Home Care Coordinator.
Are you highly organised, people-focused, and passionate about delivering outstanding care? We’re looking for a Home Care Coordinator to join our growing team and play a key role in ensuring vulnerable people in our community receive the quality care they deserve.
As a Home Care Coordinator, you’ll be at the heart of our service—balancing care delivery, staff coordination, and client satisfaction. You’ll ensure that every care package runs smoothly while supporting and motivating a dedicated team of carers.

About The Role
Are you passionate about person-centred care and supporting people to live well, safely, and independently at home?
Are you an experienced professional with strong organisational skills, looking for a new challenge?
As a Home Care Coordinator, you will play a key role in the day-to-day management of the service, ensuring high-quality care delivery across the team.
You will:
- Coordinate and support a team of Home Care Workers
- Develop a detailed understanding of each client’s needs, preferences, and wishes
- Work collaboratively with clients and colleagues to deliver personalised care
- Support, empower, coach, and motivate staff to deliver excellent standards of care
- Promote understanding of what “good care” looks like in practice
- Bring people together, remain adaptable, and demonstrate sensitivity in all interactions
You will be required to work one in four weekends on call between the hours of 6.30 - 22.30 paid in addition to your salary.

- Experience of assessing the needs and risks of vulnerable adults
- Experience of managing a team and completing staff supervisions and appraisals experience in communicating with other health care professionals
