This role is maternity cover for 12-14 months only starting from July 2026.

Hours: Mon-Fri 0900-1630 + weekday on-call on rota basis

Salary: £29,000 plus on-call, bonus scheme + benefits

Location: Wirral CH41 (Birkenhead)

About evolve

We're a specialist provider of complex care for children and adults in their own homes. Our clients include those with acquired brain injuries, physical disabilities and sensory impairments.

Our goal is to improve the lives of those in our care whilst making work a better place for our nurses and carers.

The role

You'll be working in our busy Wirral office using our rota management software and speaking to our clients, their families and our community-based care teams.

As a Care Coordinator, you're the go-to for our clients and their families to report issues, chat about staffing or sometimes just provide a good listening ear. Likewise, you're there to support our community-based staff with any help they need to deliver the highest standards of care to our clients.

You'll be supported in your role by our management team and the Lead Nurses who case-manage each package of care we provide. This is an exciting opportunity to join a new complex care provider, with opportunities to grow with the company.

Office hours are Monday-Friday 0900-1630 (35hrs/wk), with on-call currently 1 week in 3 (Monday-Thursday evening only) for which you'll be paid extra.

Requirements

What we'd like:

  • Experience in an office environment
  • Car driver with access to your own vehicle

  • Confidence in speaking to clients, families and professionals on the phone, in person and via email or text
  • Ability to work under pressure to help cover last-minute absences
  • Willingness to learn and develop your skills
