Role Responsibilities (PCN and Clinical Pharmacist):

  • Deliver high-quality care by conducting structured medication reviews and advising on medication management.
  • Offer specialist advice on pharmaceutical matters, supporting healthcare teams to optimize patient care.
  • Contribute to improving outcomes for long-term conditions and support patients with self-limiting conditions.
  • Help practices achieve key targets, such as Impact and Investment Fund (PCN Pharmacist) and QOF targets.
  • Work with patients on acute medicine requests and assist with discharge and clinic letters.

Role Requirements:

Essential:

  • Experience in general practice or working as a GP Pharmacist.

Desirable:

  • Independent prescribing qualification (required for prescribing roles, not essential for non-prescribing roles).
  • Enrolment or completion of CPPE Clinical Pharmacist Training pathway (offered for PCN Pharmacists).
  • Strong leadership, communication, and interpersonal skills.
  • Sound understanding of PCN pharmacist roles and responsibilities.
