Job Title: Respiratory/Diabetes Nurse Specialist, General Practice

Location: Remote (UK-based)

Job Type: Part-Time (2.5-3 days per week), potential to be full time in the future, if desired.

About the Role

As a Respiratory/Diabetes Nurse Specialist at Zonder, you will provide virtual care for patients managing asthma, COPD and diabetes, support self-care, and collaborate with GP practices to improve their chronic care services. Your role will include:

Clinical responsibilities

  • Conducting remote assessments and personalised care plans for patients as an autonomous practitioner.
  • Responding to asthma exacerbations within 48 hours, ensuring continuity of care.
  • Educating patients on proper inhaler techniques, medication use, and lifestyle adjustments.
  • Adhering to NICE guidelines and participating in continuous quality improvement.
  • Accurately recording clinical data in line with locally and nationally agreed guidance and templates and understand its significance.

Non-Clinical responsibilities

  • Maintaining comprehensive records of consultations within clinical systems.
  • Reporting findings that will lead to improvement in patient outcomes and/or improved processes for Zonder/GP Practice partners.
  • Working with other team members to develop and implement evidence-based clinical practice within the team through auditing user and stakeholder feedback as well as best practices developed more widely (e.g. relevant industry and charity sectors).

Who You Are

  • NMC Registered Nurse: with a minimum of 4 years in respiratory, or diabetes care, specifically in General Practice.
  • Experience in telehealth and chronic care management.
  • Knowledgeable about remote patient care and passionate about improving chronic disease management.
  • A strong communicator with the ability to work autonomously and collaboratively within a multi-disciplinary team.
  • Eager to contribute to a growing startup, share learnings, propose and try innovative approaches to care.

Desirable but not mandatory:

  • Respiratory Health Diploma, or equivalent in asthma/COPD.
  • Health Diploma, or equivalent in diabetes.
  • Independent prescriber.
