Overview

We are a CQC regulated complex care company that provides at-home TDDI nursing and complex care services for all ages.

Join our dedicated healthcare team as a Complex Care Healthcare Assistant (HCA), where your compassionate approach and clinical skills will make a meaningful difference in the lives of patients with complex health needs. In this vital role, you will work closely with multidisciplinary teams to deliver personalised care, support recovery, and promote wellbeing.

We are looking for someone to join a team or care/rehab professionals to support a young adult with acquired brain injury gain independence. The client does not require personal care but does require support with rehab and access to the community

Responsibilities

  • Must be able and willing to transport the client in your own vehicle. We would be able to help you with business insurance for this
  • Assist patients with daily activities to meet their care and rehabilitation goals in accordance with their care plans
  • Collaborate with nurses and healthcare professionals to develop, implement, and review individualized care plans tailored to each patient’s needs.
  • Monitor patient conditions vigilantly, recording vital signs and reporting any changes or concerns promptly to the clinical team.
  • Maintain clear, accurate documentation of care provided, ensuring compliance with organizational policies and legal standards.
  • Communicate effectively with patients, families, and team members to foster understanding, reassurance, and coordinated care delivery.
  • Utilize IT systems efficiently to update patient records, track care activities, and support clinical decision-making.
  • Organize workload effectively to prioritize tasks while maintaining flexibility to respond to urgent patient needs.

Requirements

  • Proven experience in rehabilitation or HCA support roles within complex or long-term care settings.
  • Strong understanding of care plans and ability to implement them accurately while adapting to changing circumstances.
  • Excellent communication skills—able to listen actively, convey information clearly, and foster positive relationships with patients and colleagues.
  • Proficiency in using IT systems for documentation and communication purposes.
  • Superior organizational skills to manage multiple responsibilities efficiently while maintaining attention to detail.
  • Ability to work independently as well as part of a multidisciplinary team in a fast-paced environment.
  • Relevant certifications or training in healthcare support are highly desirable.

Pay: £16.50-£18.00 per hour
