Through our nursing, hospice and support services, research and campaigning, we work to ensure everyone has the best possible quality of life at the end of life.
Job Description
We're excited to offer an opportunity for an experienced Clinical Nurse Specialist to join an innovative new Primary Care Network (PCN) project in Exeter.
This role will play a key part in improving how people approaching the end of life are identified, supported and connected to the care they need. Working across primary care, community and acute settings, you'll help shape a developing service that aims to improve patient outcomes, reduce unnecessary hospital admissions and support people to receive care in the place that is right for them.
This is a fantastic opportunity to influence how End of Life Care is delivered across Exeter while working collaboratively with healthcare professionals across multiple services. This role is to support proactive identification of patients, community-based care and emergency department discharge support across Exeter.
Hour: 37.5 hours per week, Monday to Friday
Contract: Permanent (flexible working to be discussed)
Salary: Band 6, £39,959 - £48,117 per annum (aligned to NHS Agenda for Change)
Location: Exeter Primary Care Network and community settings
About the role
As a Clinical Nurse Specialist, you will work closely with Primary Care Network colleagues, community services and wider multidisciplinary teams to improve the experience of people approaching the end of life.
You will:
- Support the early identification of people who may benefit from End-of-Life Care support.
- Undertake holistic clinical assessments and work collaboratively to coordinate care.
- Facilitate advance care planning discussions with patients, families and carers.
- Help improve communication and continuity of care across services.
- Support admission avoidance and contribute to effective discharge planning.
- Contribute to service development, quality improvement and best practice initiatives.
- Promote person-centred care that reflects individual preferences and priorities.

Qualifications
We're looking for a confident and compassionate registered nurse who enjoys working collaboratively and is passionate about improving patient outcomes.
You will have:
- Current NMC registration as an Adult Nurse.
- Experience supporting people with complex health needs and/or End of Life Care needs.
- Excellent clinical assessment and decision-making skills.
- Experience of care planning and coordinating care across services.
- Strong communication and relationship-building skills.
- Experience working within multidisciplinary teams.
- A commitment to delivering high-quality, person-centred care.
- An interest in service improvement and improving patient experiences.
