ReviveRise – Health Care Assistant / Support Worker
Job Summary
ReviveRise is a nurse‑led domiciliary and complex‑care provider delivering exceptional, person‑centred support across the community. We are seeking compassionate, reliable, and motivated Health Care Assistants/Support Workers to join our growing team.
In this role, you will support individuals in their own homes or care settings, promoting independence, dignity, and wellbeing. You will work under the guidance of experienced registered nurses and contribute to delivering safe, high‑quality care that makes a meaningful difference in people’s lives.
Key Responsibilities
- Provide personal care including washing, dressing, grooming, and continence support
- Assist with mobility, transfers, and safe use of equipment (e.g., hoists, stand aids)
- Support clients to maintain independence and dignity in their daily routines
- Follow individualised care plans and clinical instructions accurately
- Offer emotional support, reassurance, and companionship
- Assist with medication administration in line with training and clinical direction
- Observe, record, and report any changes in health, behaviour, or wellbeing
- Maintain accurate, timely documentation using ReviveRise digital systems
- Communicate effectively with clients, families, nurses, and the wider care team
- Support with light domestic tasks such as meal preparation and tidying where appropriate
- Uphold ReviveRise values of compassion, professionalism, and clinical excellence
Requirements
- Previous experience in home care or care‑home settings is beneficial but not essential
- Ability to follow detailed care plans and clinical instructions
- Good written and verbal English for safe communication and documentation
- Basic IT skills for electronic record keeping
- Excellent interpersonal skills and the ability to build trust with clients and colleagues
- A compassionate, patient, and respectful approach to supporting vulnerable individuals
