We are seeking an enthusiastic and motivated Project Manager to join our Integrated Care Operations Service (ICO)
This is an exciting opportunity to work at the heart of innovative integrated care programmes, supporting service delivery, operational improvement, data-driven decision-making, and partnership working across health, social care, and voluntary sector organisations.
Working closely with clinical and operational leaders, you will help shape and improve all ICO services including complex discharge, Inclusion Health patients, UCLH Hospital@Home as well as high intensity users, by providing project support and data function across the services.
This role offers the opportunity to work within a dynamic and supportive team committed to delivering innovative and integrated services. You will gain exposure to a wide range of operational, strategic, and partnership work while contributing to meaningful service transformation and improvement across the health and care system.
This role centres on supporting the delivery and improvement of of integrated, person-centred services across health , social care, and voluntary sector partners, particularly for individuals with complex needs such as care needs, homelessness, mental health challenges, and substance dependency. The postholder works collaboratively with multidisciplinary teams to implement coordinated care pathways and promote inclusion health principles, while representing the service in partnership forums and ensuring strong relationships across acute, community, and external organisations.
From a leadership and operational perspective, the role involves managing and coordinating the administrative function, overseeing workloads, and providing line management, supervision, and development for staff. It requires facilitating meetings, supporting effective communication across teams, and ensuring a positive, values-driven team culture. The postholder also monitors operational performance, manages risks, supports service delivery, and ensures issues are escalated appropriately while maintaining high standards of governance and operational resilience.
