Job summary – main purpose of the post:
Under the direction of the PBS lead you will play a vital role in ensuring that Positive Behaviour Support and the reflective practice framework are live, present and embedded within the culture at our Highfield Road Service.
Working as an individual and within a team you will ensure that PBS / person centred / active support is provided in a compassionate and respectful way to ensure the people we support are at the heart of everything we do, with a focus on improved quality of life.
Guided by the person’s PBS & support plan your role is to ensure the people we support are supported to live the life they choose, their way, by all team members. You will promote citizenship, diversity, community inclusion and equality.
Your values, personality and integrity are the key qualities that will guide your practice supplemented with professional training. You will follow best practice and adhere to all Salford Cares policies and procedures.

Support workers need to blend their personal values with those of the organisation to perform their role effectively, meet their commitments and strive to provide high quality services.
Key Responsibilities:
You will demonstrate the ability:
- To develop good working relationships and full co-operation between direct support staff and the Practice Leads and Service Management Team
- To observe and work with the team around the person we support to understand the potential causes of the risk behaviour
- To observe and work with the team around the person we support and identify opportunities to promote quality of life and important person-centred outcomes such as developing skills, promoting independence, community presence etc
- To work with the staff team to review, update and implement positive behaviour support plans as required
- To facilitate meetings with staff teams to reflect the person we support’s behavioural needs and strategies detailed in the PBS plan
- support and coach staff to embed Autism Practice, Active Support, Total Communication and PBS Culture in the service
- Communicate confidently with a range of other professionals including health professionals, GPs, Care Management, Safeguarding, voluntary groups and community groups, adapting your style as required, to promote the needs of the people who use our services.
- Ensure accurate records are maintained and share all relevant information to ensure the best outcomes for people.
- Make use of IT systems as necessary.
- Work within professional boundaries.
- Ensure that you talk to the people you support about their support, their aspirations and their life goals
You will be responsible:
- To be proactive when these practices are not being consistently used by staff and report to Team Leaders and PBS Lead when staff continually fail to adhere to these practices
- To regularly review and evaluate the success of the plans and make recommendations and /or changes as necessary
- To analyse incidents and behaviour records and identify where people or services need additional input to reduce incidents
- To complete the weekly Reflective Practice Logs, capturing information from incidents analysis and Quality of life learning logs (you will have time off shift for this)
- Alongside other Practice Leads, Team Leaders and PBS Lead, to attend monthly Reflective Practice Forums feeding back progress on person supported they are responsible for using the monthly Reflective Practice Log. Support other Practice Leads with ideas for solutions and opportunities for their person supported.
- Promote lifestyles that reflect the wishes and choices of the people we support.
- Develop and ensure you understand each person’s needs including any specialist support they require i.e. communication.
- Contribute to the safe and efficient running of premises, having a clear understanding of health and safety and using your knowledge of the person to inform risk assessments and management plans.
- Ensure that all information / plans relating to the people we support are read, understood and kept up to date.
- Understand the role of the community learning disability team in Salford and liaise with the team as necessary.
- Participate in 7 day rotas if required in the service you are employed to ensure the service is appropriately staffed according to commissioning requirements..
- Be accountable for service user finances or purchase cards as relevant to your service area and ensure accurate records are maintained.
- Be flexible to support the needs of the people we support.
- Order, stock take and manage medication.
- Complete all necessary documentation and records.
- Understand the Mental Capacity Act 2005 and Deprivation of Liberty and how this impacts on your role.
Essential Criteria:
- Experience of working in the care sector with people who have a Learning disability, Autism, Mental health needs.
- Experience of supporting individuals to increase their independence.
- Experience of working as a member of a team.
- Care Certificate or similar qualification, or willingness to work towards this.
- Able to understand and work with the team to understand the potential causes of risk behaviour
- Able to use a computer and complete all paperwork linked to PBS / Active support and Total communication approaches.
- To observe and work with a team, around the person we support, and identify opportunities to promote quality of life and important person-centred outcomes such as developing skills, promoting independence, community presence etc
- To support and coach staff to embed Autism Practice, Active Support, Total Communication and PBS Culture in the service
- Able to direct a team and be proactive when PBS / active support and total communication approaches are not being consistently used by staff and know who to report this to when staff continually fail to adhere to these practices.

Qualifications and Training
