Full job description

The Chislehurst Partnership consists of two busy doctor's surgeries based in the heart of Chislehurst. We are looking to recruit two part-time care navigators to join our existing friendly, hardworking team.

We have reviewed the role of our receptionists and think that job title 'Care navigator' more appropriately reflects the scope of the role.

This shift will be based across both sites, please see below fixed shifts each week per vacancy.

Vacancy 1

  • Monday 1pm - 6.30pm
  • Thursday 1pm - 6.30pm
  • Friday 1pm - 6.30pm

= 16.5 hours per week

Vacancy 2

  • Tuesday 1pm - 6.30pm
  • Thursday 1pm - 6.30pm
  • Friday 1pm - 6.30pm

= 16.5 hours per week

Cover and additional shifts will be available each week. All shifts would be discussed and agreed prior to working.

As part of our recruitment process, candidates will be subject to a second stage interview in the form of a trial shift before an offer is made.

Our clinical system is EMIS which integrates with AccuRx to communicate with our patients. Home visiting is supported by our practice Paramedic and Advanced Nurse Practitioner who work alongside the Bromley Healthcare District Nursing team. We are part of a PCN which has several additional practitioners including Clinical Pharmacists, Social Prescribers, a Mental Health Practitioner, First Contact Physiotherapist and Paramedic.

This role is varied and covers all aspects of general practice reception. The ideal candidate must be a team player able to adapt to changing work practices and be flexible to cover sickness and holidays. The ideal candidate must have attention to detail as accuracy is a key requirement of the role. You must also have excellent communication skills both verbal and written.

You will join an existing friendly team and provide excellent customer service. You will recognise the importance of patient services and value every patient and their needs. You will work in a team and be committed to working together with the whole practice to achieve desired outcomes.

The main duties and responsibilities of this role include :

  • To manage the flow of patients arriving at the reception desk, for appointments, prescriptions or queries.
  • Taking calls from patients and discussing symptoms to triage their medical needs, actively signposting patients to the most appropriate service.
  • Receive and make telephone calls as required. Divert calls and take messages, ensuring accuracy of detail and prompt appropriate delivery.
  • Making appointments on EMIS, by processing appointment requests for today, future appointments from patients in person and on the telephone.
  • Dealing with patient enquiries efficiently and courteously.
  • Provide administrative support to clinical team/practice team via tasks.
  • Receiving specimens from patients.
  • Keeping reception and working areas tidy, ensuring prescription paper is distributed to rooms as necessary daily and locking this away again at night.
  • Regularly checking the reception email inbox
  • First point of contact for patients and act as a focal point of communication between patients and clinical team.
  • Taking requests for home visits and ensure they are recorded and processed correctly.
  • To offer general assistance to colleagues/practice team, communicating with the clinical team to complete requests and resolve problems.
  • Maintain and manage patient records ensuring information is accurate.
  • Safeguard patient privacy and confidentiality.
  • Providing holiday and sickness cover.
  • Open/close the practice as and when necessary ensuring the premises are prepared in readiness for the morning session.
  • Anything else that is custom and practice to enable the smooth running of the Practice.
