Registered Community NurseJob Summary

Hope Homecare is seeking a compassionate, dedicated, and highly skilled Registered Community Nurse to join our growing clinical team. The successful candidate will provide high-quality nursing care to individuals within their own homes and community settings, promoting independence, dignity, and person-centred care. This role offers an excellent opportunity to make a meaningful difference in the lives of service users while working collaboratively with multidisciplinary teams across Hertfordshire and Essex.

Duties and Responsibilities

  • Deliver safe, effective, and evidence-based nursing care within community settings.
  • Conduct comprehensive nursing assessments and develop individualised care plans tailored to service users' needs.
  • Administer medications, injections, wound care, PEG feeding, catheter care, and other clinical interventions in accordance with NMC guidelines and organisational policies.
  • Monitor, evaluate, and review care plans, ensuring care remains responsive to changing needs.
  • Support individuals with long-term conditions, complex health needs, palliative care requirements, and rehabilitation goals.
  • Provide health promotion, education, and advice to service users, families, and carers.
  • Maintain accurate clinical records and documentation using electronic care management systems.
  • Identify and escalate safeguarding concerns in line with local and national safeguarding procedures.
  • Work collaboratively with GPs, hospitals, social workers, allied health professionals, and other healthcare providers to ensure continuity of care.
  • Participate in multidisciplinary meetings and contribute to service improvement initiatives.
  • Support the supervision, mentoring, and development of junior staff where appropriate.
  • Maintain professional registration and comply with all NMC standards, policies, and codes of practice.
  • Travel within the community to deliver nursing care to service users in their homes.

Requirements

  • Registered Nurse (Adult) with active NMC registration.
  • Previous experience working within community nursing, district nursing, homecare, or a similar healthcare setting.
  • Strong clinical skills including medication administration, wound care, catheter care, PEG feeding, and clinical assessments.
  • Excellent understanding of safeguarding, infection prevention and control, and clinical governance.
  • Ability to work independently and manage a caseload effectively.
  • Strong communication and interpersonal skills with the ability to build positive relationships with service users and healthcare professionals.
  • Competent in the use of electronic patient record systems and IT applications.
  • Full UK driving licence and access to a vehicle.
  • Ability to work flexibly to meet service requirements.
  • Commitment to delivering high-quality, person-centred care.

Desirable Criteria

  • Community Nursing or District Nursing experience.
  • Experience supporting individuals with complex healthcare needs.
  • Knowledge of palliative and end-of-life care.
  • Experience supervising healthcare assistants or support staff.
  • Additional clinical qualifications relevant to community nursing.
