Full job description

Our Out of Hours Service is a crucial part of our Adults Specialist Services delivering care to people who need it urgently or those with longer-term conditions. We are recruiting Band 5 Community Nurses to help us deliver the best experience possible in South Gloucestershire.

As the South Wests largest provider in community healthcare, our multi-disciplinary teams of nurses, therapists, clinicians and support colleagues help people remain safe and live as independently as possible after injury, illness or hospital admission. We take a holistic approach involving people in their care, enabling them to thrive in their homes and local communities.

And this is where you come in!

We work on a buddy system which means you will never be working alone you will always have someone with you for every visit during your shift.

In this role, you will focus on collaborative, safe, effective and evidenced-based practice. Working closely with primary and community services as well as with social care partners, you play a critical role in ensuring a person-centred experience.

Plus, with no rush hour traffic to contend with, save time on your commute, giving you more free time and flexibility during the day for a better work/life balance.

Sirona offer enhanced pay rates during evenings and weekends, full NHS benefits including salary and pension, generous annual leave and well-being support is widely available.

In this role, you will focus on collaborative, safe, effective and evidenced-based practice. Working closely with primary and community services as well as with social care partners, you play a critical role in ensuring a person-centred experience.

As an OOH Nurse, you will be required to:
Assess, plan, implement and evaluate individual, patient-centred interventions drawing on a wide range of specialist knowledge gained from practical experience and specific training.

Be competent in a broad range of clinical skills including catheterisation, pain management, wound care, end of life care and IV medication after specific training and more.

Actively use and promote shared decision-making, empowering people to improve self-management of their long-term condition while working collaboratively with our teams and system partners

Supervise colleagues including; peers, students and unregistered workforce in nursing competencies, helping us shape our existing service and contribute to the future design and delivery.

Ever dreamt about a career that sparks excitement and fosters your professional growth? Picture yourself waking up eager to make a real difference in our community, surrounded by teams who value your unique ideas. It's time to turn your dream into reality!

We are Sirona care & health – England’s largest provider of Adult’s and Children’s Community Services and we’d love for you to join us in Bristol, North Somerset or South Gloucestershire.

Agenda for Change Salary, full NHS benefits, 27+ days’ annual leave, and participation in the NHS Pension Scheme – just a glimpse of the benefits we offer.

Whether you're starting your journey in healthcare, looking for a fresh challenge within the NHS, or contemplating a career change, don't be daunted by the prospect of something different – we create development opportunities for all our colleagues and apprentices.
