Full job description
POP (Prison Offender Project) provides intense support to all prison leavers, inclusive of MAPPA (Multi Agency Public Protection Arrangements) coming through the RCT Prison Pathway. We work exclusively to help them to integrate back into their community and into long term sustainable accommodation. We work in partnership with the local authority, probation, prisons and local housing associations. We provide through the gate support when returning to the community ensuring that all mandatory appointments are attended and signposting and referrals are made to provide a seamless transition, removing possible barriers. Our aim is to prevent homelessness and reoffending. Role Summary
To provide intensive support to prison leavers returning to the RCT area with a housing need
To deliver targeted support and interventions including but not limited to
Accommodation and tenancy rights
Substance Use
Maximisation of income
Interpersonal skills
Mental and Physical health
Motivation
Employment, education and volunteering
Encouraging meaningful and supportive relationships with family
Navigating complex systems
Recovery, resilience and aftercare
To work in collaboration with HMPPS, South Wales Police, Local Housing providers, Local Authority and other partner organisations to improve the outcomes for prison leavers in RCT.
To provide through the gate support on day of release from custody and return to the RCT area
To prevent the loss of a tenancy once secured, where ever possible
Key Responsibilities
To complete full needs assessments and support plans based on indivual needs, stregths and hopes.
To accompany clients to appointment and support with independent living skills, such as shopping, budgeting and other tasks that will improve overall quality of life.
To provide harm reduction advice
To maintain and accurately document support provided, including casenotes, data base records, risk management plans and engagement
To build a port folio of key contacts with local providers and community projects
Signpost to support agencies that improve client wellbeing and both physical and mental health
To prepare reports and data analysis as and when required for Senior Managers
Attend MARAC and MAPPA meetings
To adhere to safeguarding protocols and duty to report
To complete prison vetting and key training across HMP Cardiff, HMP Swansea and HMP Parc
To support clients with homefinder, bidding and moving through the housing pathway
Support with setting up utlities, direct debits and payments once they have secured long term accommodation
Attend daily morning meetings with local temporary accommodation providers and the local authority
Build relationships with local private social landlords
Additional Responsibilities:
Follow and adhere to Kaleidoscope operational policies and procedures
Provide non judgemental support with empathy and kindness
Engage with supervision, every 4-6 weeks
Be flexible and proactive with support when needs and circumstances change
To prepare data and reports as and when required by Senior Managers and other Kaleidoscope central services team members.
Maintain and develop personal expertise and knowledge by reading, networking and further training in order to develop professional competence.
Pay: £27,000.00 per year
