We are seeking a motivated and compassionate Community Support Practitioner Worker to join our urgent community response and intermediate care team across Scarborough and Ryedale community services.

In this role, you will work closely with Physiotherapists, Occupational Therapists, and Nurses to support rehabilitation programmes and functional activities, helping patients progress with their recovery in the community.
You will support individuals with a wide range of pathologies and medical conditions who require a period of assistance to improve their mobility and independence.

You will benefit from regular supervision, development opportunities, and a comprehensive induction programme to support you in your role.

Day to day, the role includes the following care carried out in the patient's own home with supervision/delegation of therapists/ registered nurses:

Patient care following a fall

Moving & handling of patients

Vital observations

End of life care and urgent support for people living with frailty

Recognise and escalate issues of concern and/or safety

Assisting with activities of daily living, e.g., personal care tasks, meal preparation following injury/illness to prevent hospital admission- supporting and monitoring mobility, and rehabilitation programmes which include aspects of nursing, occupational Therapy and Physiotherapy.
