Locations: Neath Port Talbot & Bridgend County (Community-Based / On the Road)

Job Type: Full-time / Part-time / Permanent

Pay: £13.45 – £13.75 per hour (Equivalent to £27,976 – £28,600 per year based on a 40-hour week)

As a Community Care & Support Worker, you'll provide person-centred support that focuses on helping people achieve the outcomes that matter most to them.

Outcome-Focused Support

  • Support individuals to achieve personal goals, maximise independence, and maintain meaningful daily routines.

Daily Living Support

  • Provide compassionate support with personal care, medication, meal preparation, household tasks, and other everyday activities.

Digital Care Recording

  • Use our secure mobile app to complete care records, monitor outcomes, and communicate effectively with colleagues.

Relationship Building

  • Develop positive, respectful relationships with the people you support, their families, and professionals involved in their care.

Safety & Quality

  • Follow safeguarding procedures, risk assessments, and safe working practices, raising concerns promptly where required.

Requirements

Essential

  • Full UK Driving Licence.
  • Access to your own vehicle with business insurance.
  • Willingness to use a smartphone for digital care recording.
  • Flexibility to work a rota that includes evenings, weekends, and bank holidays.
  • A caring, compassionate and professional approach, with a genuine desire to improve people's lives.

Desirable (but not essential)

  • Around one year's experience in social care or support work.
  • QCF Level 2 (or equivalent) in Health & Social Care.
  • Knowledge of the Social Services and Well-being (Wales) Act 2014.
