Pay: £13.45 – £13.75 per hour (Equivalent to £27,976 – £28,600 per year based on a 40-hour week)

Critical Application Criteria

Drivers Only: You must hold a full UK Driving Licence, have daily access to your own reliable vehicle, and have business insurance for community travel. Unfortunately, we are unable to consider non-drivers for this role.

The Role

As a Community Care & Support Worker, you'll provide person-centred support that focuses on helping people achieve the outcomes that matter most to them.

This might include supporting someone to:

• Regain the confidence to shop independently.
• Maintain daily routines within their own home.
• Safely access their local community.
• Build meaningful social connections.
• Develop the skills and confidence to live as independently as possible.

Every interaction is delivered in a way that promotes dignity, choice, control, and wellbeing.

Key Responsibilities

Outcome-Focused Support

• Support individuals to achieve personal goals, maximise independence, and maintain meaningful daily routines.

Daily Living Support

• Provide compassionate support with personal care, medication, meal preparation, household tasks, and other everyday activities.

Digital Care Recording

• Use our secure mobile app to complete care records, monitor outcomes, and communicate effectively with colleagues.

Relationship Building

• Develop positive, respectful relationships with the people you support, their families, and professionals involved in their care.

Safety & Quality

• Follow safeguarding procedures, risk assessments, and safe working practices, raising concerns promptly where required.

Requirements

Essential

• Full UK Driving Licence.
• Access to your own vehicle with business insurance.
• Willingness to use a smartphone for digital care recording.
• Flexibility to work a rota that includes evenings, weekends, and bank holidays.
• A caring, compassionate and professional approach, with a genuine desire to improve people's lives.

Desirable (but not essential)

• Around one year's experience in social care or support work.
• QCF Level 2 (or equivalent) in Health & Social Care.
• Knowledge of the Social Services and Well-being (Wales) Act 2014.
