Pathway Referral & Volunteer Support worker

(Est. 10 hours Pathway refferal / 4 hours volunteering coordinator support)

Responsible to: Line Manager & Trustees

Location: Welshpool Community Haven, Ann Holloway Centre, Welshpool, with outreach across Powys where required.

Hours: 12–14 hours per week

Salary: £14 - £16.00 per hour (Depending on experiance)

Contract: Fixed Term to April 2027 (subject to funding) with hope to extend after to permenant role

Job Purpose

The Pathway Referral & Family Support Worker will provide a welcoming first point of contact for individuals and families requiring additional support following diagnosis or becoming a carer.

The postholder will work alongside individuals, carers and families to identify their needs, provide practical information, advice and support, and connect them with appropriate statutory, voluntary and community services.

The role aims to reduce isolation, improve wellbeing, promote independence and prevent people's needs from escalating by ensuring they receive the right support at the right time.

The postholder will work closely with partner organisations across Powys to strengthen community connections and improve access to local services.

Key ResponsibilitiesReferral Coordination

  • Receive referrals from health professionals, social care, schools, community organisations and self-referrals.
  • Undertake initial conversations and assessments of need.
  • Hold drop in sessions
  • Develop person-centred support
  • Signpost and refer individuals to appropriate services.

Family Support

Provide practical support to families and carers including:

  • Information and advice
  • Emotional support
  • Advocacy where appropriate
  • Support with completing forms and applications
  • Assistance accessing benefits and financial support
  • Help accessing health and social care services
  • Carer support and signposting
  • Support during periods of crisis or change

The role will empower families to make informed decisions and increase their confidence in accessing services independently.

Promote the Pathway Referral Service through:

Administration

Maintain accurate confidential records including:

  • Referral records
  • Case notes
  • Support plans
  • Outcome monitoring

Ensure all records comply with GDPR and organisational policies.

  • Experience supporting vulnerable individuals or families.
  • Experience working within health, social care or the third sector.
