Ready to take the next step in your career and lead from the front? This is a fantastic opportunity for an experienced Social Worker or Team Manager to step into a pivotal leadership role within a Hospital Discharge Service.

You’ll also benefit from hybrid working, with the flexibility to work from home up to two days per week – helping you maintain a healthy balance between leadership responsibilities and life outside of work.

As this is a permanent role we understand how tedious it can be waiting for local authority onboarding process to be finalised, that's why we not only do all the liaising on your behalf with managers, but we also work to help get you into your new role as fast as possible.

In this role, you’ll oversee a busy service supporting safe and timely discharges across a 200-bed pathway. You’ll play a key role in allocating cases, monitoring performance, and ensuring that assessments and reviews are completed within expected timeframes – including 14–28 day assessments and 6–12 week reviews.

This is a hands-on leadership role where you’ll guide and support your team through a light-touch triage system, ensuring workflows are efficient and outcomes remain person-centred. You’ll be instrumental in maintaining high standards of practice while supporting staff with decision-making, prioritisation, and complex cases.

You’ll be working alongside a strong multidisciplinary network, helping to drive effective discharge planning and ensuring individuals are supported to return home safely with the right level of care in place.

You’ll join a service that values collaboration, structure, and supportive management – where your leadership style can make a real difference to both team morale and service delivery.

As this is a statutory role, you must be a qualified Social Worker and registered with Social Work England, with experience in supervising or managing within adult social care.
