We formally review performance & pay in our colleagues' annual reviews and also regularly benchmark our salaries.
Rarely make sure you record sessions like I did so you have proof of what was discussed. Appraisal form changed and sent to HR after discussed with me. I refused to sign so was sacked.
Fairly often, to roll your targets forward, making them unachievable and rolling your probation date so you don't get your pay rise.
Annually and will be bare minimum
