Full job description

Job Purpose: The General Medical Practitioner (GMP) will provide high-quality, patient-centred care to individuals and families within the Primary Care Network. The GMP will be responsible for diagnosing and treating a range of medical conditions, promoting health and well-being, and working collaboratively with other healthcare professionals to enhance patient outcomes. Key Responsibilities: Conduct comprehensive assessments and examinations of patients. Diagnose and manage acute and chronic medical conditions.

Develop and implement individualised care plans in collaboration with patients and the healthcare team. Prescribe medications and therapies as appropriate. Provide preventive care and health education to patients. Participate in multidisciplinary team meetings and case discussions.

Maintain accurate and up-to-date patient records in accordance with legal and ethical standards. Engage in continuous professional development and training. Support the PCN in achieving health targets and initiatives. Essential Skills: GMC registration and a valid license to practice.

Strong clinical assessment and diagnostic skills. Excellent communication and interpersonal skills. Ability to work collaboratively within a multidisciplinary team. Good organisational and time management skills.

Commitment to ongoing professional development. Desirable Skills: Experience in a primary care setting. Special interests in specific areas of medicine (e.g., mental health, chronic disease management). Familiarity with electronic health record systems.

Knowledge of current NHS policies and guidelines.
