The Newport Neighbourhood Care Network has an exciting opportunity for two dynamic, forward thinking MDT Coordinators to work with our GP Practices across Newport.

We are committed to developing the Place Based Care Service to ensure that holistic patient centred care is delivered as close to home as is possible and supports people’s health and well being whilst promoting prevention and self-management opportunities.

The MDT Coordinator will be the main point of contact for Newport GP Practices and partner agencies such as Newport City Council, GAVO and Age Cymru.

The post holder will be responsible for planning and coordinating the smooth running of GP Practice MDTs, where they will be responsible for allocating and re-allocating tasks, recording performance outcomes to implement and embed a sustainable, localised model of place-based care that enhances patient centred care to our more vulnerable patients.

This opportunity is fixed term to 31/3/27
