Full job description
Please note: at this stage, the role is open only to colleagues currently employed within Kent and Medway NHS Trusts, the Kent and Medway Integrated Care Board (ICB) and Medway Community Healthcare or working on the Staff Bank within these organisations. Nursing and AHP vacancies are also open to Student Nurses and Newly Qualified Nurses/AHPs.
Join our West Kent Urgent Care Service as a Twilight Nurse and make a real difference when patients need support most. Our team provides a rapid two-hour crisis response, helping people remain safely in their own homes, avoiding unnecessary hospital admissions and supporting timely discharge from acute settings.
Working across a 24/7 service, you'll deliver compassionate, responsive care to patients with urgent, complex and end-of-life needs. Using a wide range of clinical skills, you'll provide treatments often associated with hospital care, including IV therapy, syringe driver management, catheterisation, phlebotomy, wound care and clinical assessment.
This is a rewarding opportunity for a confident and caring nurse who enjoys autonomous working while being part of a supportive, experienced multidisciplinary team. You'll collaborate closely with health and social care colleagues to deliver high-quality, patient-centred care that has a direct impact on people's lives during times of crisis.
Please note, this vacancy does not meet the criteria for skilled worker sponsorship. Therefore, we are unable to accept applications from candidates that cannot provide documentary evidence of right to work in the United Kingdom.
As a valued member of our community nursing team, you'll provide holistic, person-centred care to patients with diverse and often complex healthcare needs.
You'll undertake comprehensive assessments, develop and review personalised care plans, and work in partnership with patients, families and wider professionals to achieve the best outcomes. You'll coordinate care, arrange equipment, make onward referrals and deliver a range of clinical interventions while monitoring patients for signs of deterioration and responding appropriately.
The role also includes managing a daily caseload, supporting triage and prioritisation of referrals, and contributing to the smooth running of the service. You'll play an important part in developing others by supervising and supporting healthcare assistants, students and junior colleagues.
This role is ideal for a proactive nurse who thrives in a community setting, values teamwork and is passionate about delivering outstanding care closer to home.
Kent Community Health NHS Foundation Trust is currently undertaking a significant programme of transformation to align our community services to the new Neighbourhood Model of care delivery, there will be a period of consultation which may result in changes affecting some services, teams or roles. Depending on the area you are applying to work in, this could include potential changes to your role, work base, reporting arrangements, or terms and conditions of employment. Any changes would be subject to consultation and managed sensitively and in line with relevant employment processes. Please do contact the recruiting manager if you have any concerns about this so they can answer any questions you might have.
Rated ‘Outstanding’ by CQC, we deliver high-quality care that improves the health of our communities. We are looking for people who share our values: Compassionate, Aspirational, Responsive and Excellent .
