Lakeside Healthcare is a large GP partnership that provides NHS care for around 170,000 patients across Cambridgeshire, Peterborough, Northamptonshire and Lincolnshire. We are made of eight GP practices across 14 sites that are part of their local communities.

About the Practice

An exciting opportunity has arisen to join our forward thinking, teaching practice located in Stamford Lincolnshire. We are currently expanding our team and are looking for Nurse Practitioner, Emergency Care Practitioner or Advanced Nurse Practitioner to join our team.

Lakeside Healthcare at Stamford is part of the Lakeside Healthcare partnership, which serves the healthcare needs of over 165,000 patients across four counties- Cambridgeshire, Lincolnshire, Northamptonshire and Rutland.

Stamford is a well-established surgery with a list size approximately of 28,000 patients. We achieved QOF outcomes with the support of a well-established QOF team. The surgery offers access by several routes, digital platform as well as telephone, video, and face to face consultation. There is a daily Duty team with a dedicated GP, NP/ANP and HCA daily.

You will be supported by a multidisciplinary team of currently, 4 NP/ANPs, 5 Practice nurses, 5 Healthcare assistants, 1 General Practice Assistant, Mental Health Nurse, Senior Pharmacist/Pharmacists, Pharmacy technicians and dispensing team as well as a Neighbourhood Team and Social Prescribers.

We are looking for an experienced and motivated Nurse Practitioner, Emergency Contact Practitioner, Advanced Nurse Practitioner to join our dynamic and working closely with the leadership and practice teams. You’ll be involved in the smooth day to day running of the practice and pro-actively engaged in assisting both practices to achieve its vision and objectives.

What you will do

NP/ECP/ANPs operating in primary care have a wide range of skills, a broad knowledge base, and the ability to deliver specific aspects of care. At times these will need to be supplemented by the skills of specialist health care professionals in both primary and secondary care and include use of a range of diagnostic and screening services. Although independent practitioners, the act collaboratively with colleagues working in the same area of practice or refer to and share care with colleagues in more specialist areas.

In summary, the post holder is an experienced Healthcare Practitioner who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice.

The post holder will demonstrate critical thinking in the clinical decision-making process. They will work collaboratively with a range of multi-professional teams to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required. To work at this level, NMC /HCPC requirements for advanced practice must be met.

  • An ability to make professional autonomous decisions for which they are accountable.
  • An ability to assess, diagnose and treat patients with undifferentiated and undiagnosed problems.
  • Screen patients for disease risk factors and early signs of illness.
  • Make differential diagnosis using decision-making and problem-solving skills.
  • Develop, with the patient, an ongoing care plan with emphasis on health education and preventative measures.
  • Order and follow up on necessary investigations providing treatment and care both individually, as part of a primary care team and through referral to other agencies.
  • Work collaboratively with other health care professionals and disciplines.
  • An ability to provide leadership, education and mentorship.
  • An ability to provide clear and accurate documentation.
  • An appreciation of the importance of confidentiality.
  • The ability to assess for safeguarding issues and proactively refer on to relevant organisations / safeguarding teams.
  • An acknowledgement of the importance of equality, diversity and the rights of patients, carers and colleagues. The ANP will demonstrate a non-judgmental and respectful approach to each individual’s circumstances, feelings, beliefs, priorities and rights.

  • RGN Qualification
  • NMC 1st level Registration
  • Educated to Masters Level – Nurse Practitioner Pathway.
  • Independent Prescribing Qualification or working towards.
  • Evidence of continued professional development

Desirable

  • Clinical Supervision training or keen to work towards.
  • Accredited training in chronic disease management or keen to work towards.
