Forest Heath Primary Care Network (FHPCN) is offering an exciting opportunity for a dedicated and organised individual to join our innovative team as a Care Coordinator (Administrator) Support. This role supports the reshaping of primary care across our vibrant and growing community.
You will be based primarily at Market Cross Surgery, Mildenhall, with the potential to work across other GP practices within the PCN, including: Brandon Medical Practice, Forest Surgery, The Surgery – Lakenheath, Oakfield Surgery, Orchard House Surgery, The Reynard Surgery, and Rookery Medical Centre.

FHPCN is the largest Primary Care Network in West Suffolk, serving a population of over 72,000 patients across eight GP practices in the Newmarket, Brandon, Mildenhall, and Lakenheath areas. We are a dynamic and supportive network committed to staff development and delivering high-quality, accessible care.
Role Overview
As a Care Coordinator (Administrator) Support, you will play a vital role in delivering efficient, timely, and patient-centred care. You’ll coordinate key aspects of the patient journey, assist with personalised care planning, and help patients access services that meet their health and wellbeing needs.
This is a highly collaborative role, working closely with GPs, Social Prescribing Link Workers, Health and Wellbeing Coaches, and wider multidisciplinary teams (MDT) across the PCN. You will also support digital initiatives, organise meetings, and contribute to continuous service improvement.
This role is funded through the Additional Roles Reimbursement Scheme (ARRS). You must be enrolled in, undertaking, or have completed relevant training as outlined by the Personalised Care Institute and the Workforce Development Framework for Care Coordinators, including a Level 3 qualification or equivalent.
Key Responsibilities
Access, Planning & Coordination
- Coordinate activities to support patient access to health and care services
- Ensure timely and person-centred care planning and proactive follow-up.
- Organise and support internal and cross-practice meetings, including MDTs.
- Support digital and administrative initiatives to improve patient experience.
Personalised Care Delivery
- Use data and clinical insight to identify patients who may benefit from care coordination.
- Develop and deliver Personalised Care and Support Plans (PCSPs).
- Assist patients in preparing for shared decision-making conversations.
- Provide accessible information to empower informed decision-making.
Navigation and Support
- Help patients navigate the health, care, and voluntary sector systems.
- Work alongside the Social Prescribing team and other PCN roles to deliver holistic care.
- Manage appointments and patient queries to support seamless service access.
Empowerment and Self-Management
- Promote tools and services that help patients manage their own health and wellbeing.
- Support access to peer groups, educational resources, and personal health budgets.
- Facilitate referrals to community-based training, benefits, and employment support.
Collaboration and Communication
- Be an active contributor to PCN-wide Multi-Disciplinary Team (MDT) working.
- Liaise with primary care teams to ensure integrated and coordinated support.
- Raise awareness of shared decision-making among staff and patients.
Wider Responsibilities
- Ensure referrals are made to safe, inclusive services that foster wellbeing.
- Maintain knowledge of local community resources and services.
- Escalate safeguarding concerns in line with Practice/PCN policies.
Knowledge, Skills and Experience
Essential:
- Experience working with individuals with long-term conditions or in health, care, or support services.
- Excellent communication and interpersonal skills.
- Ability to work effectively with a wide range of professionals in MDT settings.
- Proficient in managing patient records and sensitive information confidentially.
- Strong administrative and organisational abilities with great attention to detail.
- Proficiency in Microsoft Office (Word, Excel, Outlook, PowerPoint).
- Empathetic and person-centred approach, motivated by helping others.
Desirable:
- Previous experience in care coordination, social prescribing, or similar roles.
- Understanding of personalised care and shared decision-making principles.
- Experience in care planning, case coordination, or service navigation.
- Knowledge of local support services, community and voluntary sector organisations.
- Experience working with vulnerable groups and awareness of safeguarding processes.
- Experience working in primary care, care homes, or community-based settings.
- Level 2 or above in Health and Social Care (or equivalent qualification).
- Local knowledge of the Forest Heath area and willingness to travel between sites (car driver preferred).
