Lakeside Healthcare is a large GP partnership that provides NHS care for around 170,000 patients across Cambridgeshire, Peterborough, Northamptonshire and Lincolnshire. We are made of eight GP practices across 14 sites that are part of their local communities.

We work together as practices to provide resilience, sharing learning and professional back-office support services.

About the Practice/Department/Team

Lakeside Healthcare lead four Primary Care Networks. Along with three other members of this team, the post holder will support our Four Counties PCN.

Since the inception of PCNs in 2019, we have witnessed remarkable growth and success stories within our PCNs, particularly in personalised care, pharmacy services, and the diverse clinical staff meeting the unique needs of our patients.

The role of PCNs in primary care and integrated neighbourhood healthcare is continually expanding, to adapt to this growth and align with the evolving landscape, we are expanding our teams to ensure we have the capacity to support our PCNs in delivering effective services for our patients, in partnership with our practices.

The role is key to our ambition and is part of a wider PCN support team of ten people.

Job Introduction

The Digital & Transformation Manager will support the leadership of the Four Counties PCN but also work across all Lakeside Healthcare led PCNs to support with projects appropriate to their skills.

The post holder will work with clinical and operational teams to ensure the PCN contract is delivered to standards in Four Counties, deliver service improvement and efficiency, identify opportunities to make tangible positive transformation and execute change in a collaborative way.

The role holder will be key to the vision of the Four Counties PCN but also contribute to the vision for all Lakeside Healthcare led PCNs.

What you will do

1. Ensure the PCN contract is delivered to standard.

2. Work closely with staff and management working at practice and PCN level, to understand current service models, identify challenges, agree priorities for improvement and then support these to be delivered.

3. Implement, embed and evaluate practical improvements that make a difference to patients and staff.

4. Support project implementation and embedding, Quality Improvement (QI) processes, and coordinate and directly support the delivery of transformation activity at Four Counties PCN across the PCNs as appropriate.

5. Optimise the use of technology.

6. Address health inequalities and championing diversity, inclusion, and sustainability of general practice.

Essential Criteria

1. Leadership and management experience.

2. Experience of project management, with familiarity around data drive quality improvement processes.

3. Excellent communication and presentation skills, with the ability to communicate complex issues to a variety of audiences.
