JOB TITLE: Physical Activity Referral Specialist (PARS) - Move Together Buckinghamshire

DEPARTMENT: Sports Development and Physical Activity

REPORTS: Physical Activity Manager

SALARY: £23,000 - £26,000 per annum depending on experience. Full time role. Interested candidates should submit a CV (part time roles may be considered for the right candidate)

Location: Hybrid/flexible role with dedicated office days, days offsite visiting patients and work from home days.

MAIN ROLE FOCUS:

We are looking for an exercise referral Specialist (PARS) to be part of our team delivering the Physical Activity Referral Service, Move Together Buckinghamshire (MTB). You will be working with a range of partners, from GP practices to organisations like Age UK across Aylesbury, Chesham and Wycombe.

You will be working with inactive adults over the age of 50 with a long term health condition to become more physically active by working with them to create a person centered 12 week programme aimed at making it easier for people to move more.

The successful candidate will conduct initial assessments, create a 12 week movement plan and monitor the impact of the plan on the individuals referred through the pathway.

Key Responsibilities

  • Receive referrals through the newly commissioned JOY case management system
  • Complete a thorough initial assessment with referred individuals to understand current barriers to physical activity, ability to move more and learn what the individual would like to do to be more active
  • Using the information and trust gained in the initial assessment, create a bespoke, person centered 12 week plan to increase the physical activity levels of the individual
  • 12 week programmes will need to provide guidance based on the individual's preferences and demonstrate an understanding of the barriers the individual might face
  • Programmes should have a clear aim of increasing movement but with an awareness programmes will look different for each individual. For some, traditional physical activity may not be appropriate so PARS should be capable of and motivated to find alternative forms of activity such as gardening groups, walking groups or home based adapted exercise
  • PARS are required to keep the JOY system updated throughout the clients journey to allow for accurate monitoring of progress
  • PARS will work towards a target time per client and will be aware of overall target numbers of individuals seen per week, month and year
  • PARS will provide 3 “check in’s” with each individual throughout the course of the 12 week programme, ensuring the programme remains focused on meeting the individual's needs
  • Provide a progressive menu-based approach, including home activity support, digital options, community activity and specialist exercise interventions to maintain activity, offering alternative solutions to barriers or opportunities where required (all resident-led), to enable a long-term sustained increase in activity level.
  • Exploring the local provision to find opportunities for classes to refer into
  • Feedback to the project manager when gaps in provision are identified. This may be a lack of seated exercise classes in a certain geographic area, or requests from residents for a certain type of activity that is not currently provided for locally.
  • Promote the service to local communities and organisations to support ongoing referrals into the service

Person Specification

Essential

  • Physical Activity Specialists must be as a minimum:
  • Level 2 exercise on referral/personal training qualified
  • Willing to undertake further professional development (CPD) where beneficial to the pathway
  • Driving and ownership of your own vehicle is essential, with travel around Aylesbury, Wycombe and Chesham required on occasion
  • An understanding on the importance of physical activity to people's health.
  • Ability to work as part of a team and self-motivation to work individually.
  • Strong organisational skills and attention to detail.
  • Excellent interpersonal skills and the ability to establish and maintain good working relationships.
  • Highly organised with ability to prioritise a heavy workload, manage tasks simultaneously and perform effectively under pressure.
  • Understanding of the barriers and motivations for older adults around physical activity, their needs and potential issues.
  • Aware and able to provide culturally sensitive physical activity advice.

Desirable

  • Level 3 Exercise on Referral qualified
  • 1 year supporting adults with long term conditions through Exercise on Referral
  • Mental Health First Aid Training
  • Health Coaching qualification
  • Qualified in behaviour change theory and implementing the COM-B methods.

HOURS AND BENEFITS:

  • 38 hours a week (flexible hours available)
