    Social Prescribing Link Worker – Asthma (Children and Young People)

    In North East Lincolnshire, we view the health and wellbeing of young people as fundamental to leading a healthy adult life. This role is dedicated to supporting young individuals and their families in improving the health and wellness of children with asthma.

    Social Prescribing Link Workers connect children and young people with local community activities and services that can help improve their health and wellbeing, empowering by focussing on what matters most, to them.

    The Social Prescribing Link Worker is central to developing a relationship with participants and can help to strengthen community and personal resilience, and reduce health inequalities by addressing the wider determinants of health, such as debt, poor housing, and physical inactivity; and by increasing people’s active involvement with their local communities. It particularly works for children and young people with long term health conditions, those who are lonely or isolated, or have complex social needs which affect their wellbeing.

    Participants will be supported by their Social Prescribing Link Worker throughout our programme. The Link Worker will develop and manage their own caseload to ensure that reviews are carried out on time and there is regular contact with their participants

    The role will be responsible for creating an action plan with participants to achieve their desired outcomes, and support them through delivery of the action plan, with staged assessments and redirection where appropriate.

    You should be proactive and have the ability to be resourceful and creative to find the best support for participants and liaise with external organisations, build partner relationships, and attend events to promote the programme.

    Working closely with Primary care settings and school nurses to promote the Asthma Friendly School initiative and encouraging schools to participate. Not only helping children who suffer with Asthma but making the school a safer and healthier place to be for all.

    You will need a positive, ‘can do’ approach, working individually and as part of a team, communicating with people at all levels. You should have experience of working positively with children and young people facing social and emotional challenges, and knowledge of solution-focussed coaching approaches. You will be working with people from diverse cultural and social backgrounds and will need to work confidently and effectively in a diverse and challenging environment.

    Accurate and safe record keeping is essential to this role, and you will be expected to record actions and events as required in line with current legislation.

    We are looking for a friendly, engaging, empathetic individual with a natural ability to deliver with a compassionate and practical approach, putting the participant at the centre of the programme.

    Effective communication with children and their families is crucial for bringing about positive changes in a child's health. It's important to be flexible and considerate by scheduling appointments and interventions around the child's school or college hours.

    You will have strong organisational skills, and excellent verbal, and written communication skills. The ability to quickly build positive and supportive relationships. The ability to work with IT systems, input, and export accurate information and a professional approach to the presentation of information and communication. A strong commitment to team working with a proven ability to work flexibly as part of a team.

    You will be required to undertake home visits so you will be required to hold a full clean driving licence and access to your own vehicle with insurance coverage for business use.

    Accountable for: Supporting participants through a Social Prescribing programme

    Salary: £25,859.93 p.a – FIXED TERM ROLE FOR 12 MONTHS

    Hours: 37.5 hours a week, you will need to be flexible to work with the team to cover the operational hours of 8am and 7pm Monday – Friday, 9am and 2pm Saturdays with occasional other hours as needed.

    Place of work: Centre4, 17 Wootton Road, Grimsby, DN33 1HE and other sites as needed, including home visits

    Annual leave: 25 days/year plus bank holidays pro rata, rising to 30 days/year with two years of service

    Key Tasks and Responsibilities

    • Work as part of a team to deliver an effective Social Prescribing Service to participants and GPs across North East Lincolnshire.
    • Work closely with GP practices, health care practitioners and partners to bridge the gap between clinical services and Social Prescribing.
    • Meet with participants and carry out home/community visits as required.
    • Undertake person-centred assessments to co-design health and well-being action plans with individual participants and identify support needs to ensure maximum engagement and improved health and well-being.
    • Provide participants with continuity of support, motivation and a co-ordinated experience of care, remaining the point of contact throughout the person’s Social Prescribing journey.
    • Establish and maintain effective liaison with stakeholders including health, voluntary, social and education resources by attending relevant meetings and forums as required to promote the service.
    • Work in partnership with voluntary and community sector organisations to understand local resources available to support an effective Social Prescription.
    • Ensure information on sources of voluntary and community support is up to date to enable effective and accurate signposting by connecting service users with appropriate support and activities.
    • Maintain accurate and comprehensive case notes and evaluation systems, including the Wellbeing Star and other IT recording systems as required.
    • Participate in meetings including representation from stakeholder groups to support the on-going development, monitoring, and evaluation of the programme.
    • To work with schools and the school nurses in increasing the knowledge and take up of the Asthma friendly school initiative
    • Adhere to confidentiality and information sharing protocols and design comprehensive outcome focused reports detailing progress of participants.
    • Work flexibly as required and engage with Centre4 meetings and activities to promote, support, and celebrate the work of Centre4.
