Wigan Cares’ is recruiting a Support Coordinator to provide leadership and management to teams of Support Workers within the borough of Wigan. As a Support Coordinator you will facilitate and lead the delivery of high quality, compassionate care, and support to enable the people we support to lead valued lives as members of their communities. We are a person centred team supporting individuals with learning disabilities and / or an autistic spectrum condition to live in their own homes.

About the Support Coordinator role:
As the Support Coordinator you will be managing teams of Support Workers who support people in their own homes within a supported living environment. The people we support are adults who have a learning disability and / or an autistic spectrum condition. Some of the people who use our services also have significant physical health and personal care needs.
The service operates 24 hours a day, 365 days a year, and full time you will work 37.5 hours each week which could include weekends and sleep-in cover. You will also be required to participate on an out of hours management on call rota.
Responsibilities:
- Staff support, management, and supervision
- Rota management including managing staff sickness and annual leave.
- Participate in on call management rota.
- Ensuring services are person centred and deliver positive outcomes for the people we support.
- To motivate staff teams to deliver support using the active support model.
- Supporting teams to support behaviours of concern using the positive behaviour support framework.
- Working with people with a variety of care and support needs
- Supporting and enabling people to live more independently and undertake life enhancing activities.
- Ensure people’s needs are at the heart of everything you do.
- Working with care managers and health colleagues to implement and support therapy plans.
- Personal Care as required.
- Connecting with the community
- Partnership working with housing providers.

Experience and skills required:
Management experience preferably in a social care setting
- Knowledge and experience of supporting individuals with a learning disability and / or autistic spectrum condition.
- Be able to develop supportive relationships and be passionate about promoting independence and wellbeing.
- You will be highly motivated to deliver outstanding care and support.
- You must be able to participate in an on-call rota.
