Job Summary
The Care Coordinator plays a vital role in ensuring that patients receive comprehensive and effective care. This position involves collaborating with healthcare professionals, patients, and their families to facilitate communication and coordinate services. The ideal candidate will possess strong organisational skills and a compassionate approach to patient care.

Responsibilities

  • Serve as the primary point of contact for patients and their families, addressing inquiries and concerns regarding care plans.
  • Coordinate appointments, referrals, and follow-up care to ensure continuity of services.
  • Maintain accurate and up-to-date patient records, ensuring compliance with healthcare regulations.
  • Collaborate with multidisciplinary teams to develop and implement individualised care plans tailored to each patient's needs.
  • Monitor patient progress and adjust care plans as necessary, communicating changes to all relevant parties.
  • Assist in the management of office operations, including scheduling, filing, and maintaining supplies.
  • Provide support in navigating healthcare systems and resources available to patients.
