Liquid Personnel is recruiting Social Worker for its client’s Adults Discharge to Assess Team at Bristol. This is a fantastic opportunity to support citizens during a critical stage in their care journey, ensuring safe and effective transitions from hospital to community settings.


What will your responsibilities be?

  • Assess and support citizens on discharge from hospital across three discharge pathways
  • Complete post-discharge assessments and reviews in line with the Care Act
  • Undertake Care Act Assessments (CAA) and support planning
  • Carry out Best Interest (BI) decisions and work within the Mental Capacity Act (MCA)
  • Manage safeguarding concerns in line with statutory responsibilities
  • Provide duty cover where required
  • Attend Quality Assurance (QA) panels to support funding decisions

Qualifications & Experience:

To be successful in this role you must have,

  • Qualified Social Worker with relevant Social Work England registration
  • Experience within Adults Services, ideally discharge or hospital-based teams
  • Strong knowledge of Care Act, MCA, and safeguarding legislation
  • Ability to work in a fast-paced environment and manage competing priorities
