Liquid Personnel is recruiting Social Worker for its client’s Adults Discharge to Assess Team at Bristol. This is a fantastic opportunity to support citizens during a critical stage in their care journey, ensuring safe and effective transitions from hospital to community settings.
What will your responsibilities be?
- Assess and support citizens on discharge from hospital across three discharge pathways
- Complete post-discharge assessments and reviews in line with the Care Act
- Undertake Care Act Assessments (CAA) and support planning
- Carry out Best Interest (BI) decisions and work within the Mental Capacity Act (MCA)
- Manage safeguarding concerns in line with statutory responsibilities
- Provide duty cover where required
- Attend Quality Assurance (QA) panels to support funding decisions

Qualifications & Experience:
To be successful in this role you must have,
- Qualified Social Worker with relevant Social Work England registration
- Experience within Adults Services, ideally discharge or hospital-based teams
- Strong knowledge of Care Act, MCA, and safeguarding legislation
- Ability to work in a fast-paced environment and manage competing priorities
