Full job description

We are looking for a GPhC-registered Clinical Pharmacist to support an established anticoagulation service from 1st January, with fully flexible session times and dates to fit around your existing commitments. Ideal for an experienced clinical or primary care pharmacist confident in managing warfarin and DOAC therapy in a community or primary care setting.

Responsibilities:

  • Deliver anticoagulation clinics, including INR monitoring, warfarin and DOAC dose adjustments, and patient counselling in line with local protocols.
  • Review blood results, optimise anticoagulant therapy, and document outcomes clearly in the clinical system.
  • Work collaboratively with GPs, nurses, and the wider MDT to ensure safe and effective anticoagulation management.
  • Contribute to medicines optimisation, medication safety, and adherence to local and national anticoagulation guidelines.

  • Paid via LTD company or sole trader (self-employed).

Requirements:

  • GPhC-registered Pharmacist (clinical/primary care experience in anticoagulation strongly preferred).
  • Confident managing warfarin and DOACs, interpreting INR and relevant blood tests.
  • Able to work independently within local protocols and as part of a multidisciplinary team.
  • Excellent communication and documentation skills.
