Role Overview

We are seeking a qualified and passionate Social Worker to provide professional support to service users, helping them access the care, resources, and advocacy they need. You will work closely with clients, families, multi-disciplinary professionals, and internal teams to ensure that each person receives thoughtful and appropriate care.

Key Responsibilities

  • Conduct assessments to identify client needs and develop appropriate care plans
  • Provide guidance, emotional support, and practical solutions to individuals and families
  • Work collaboratively with health professionals, local authorities, and external agencies
  • Maintain accurate, up-to-date case records and documentation
  • Safeguard vulnerable individuals and ensure all actions meet regulatory and legal standards
  • Support clients through crises, ensuring timely interventions
  • Promote independence, wellbeing, and access to community resources

Requirements

  • Qualified Social Worker (Degree in Social Work or equivalent)
  • Registered with Social Work England
  • Experience working in health, social care, or community settings
  • Strong communication, assessment, and problem-solving skills
  • Ability to work independently and as part of a team
  • A compassionate and patient-centred approach
  • Full UK driving licence (preferred but not essential)
