Role Overview
We are seeking a qualified and passionate Social Worker to provide professional support to service users, helping them access the care, resources, and advocacy they need. You will work closely with clients, families, multi-disciplinary professionals, and internal teams to ensure that each person receives thoughtful and appropriate care.
Key Responsibilities
- Conduct assessments to identify client needs and develop appropriate care plans
- Provide guidance, emotional support, and practical solutions to individuals and families
- Work collaboratively with health professionals, local authorities, and external agencies
- Maintain accurate, up-to-date case records and documentation
- Safeguard vulnerable individuals and ensure all actions meet regulatory and legal standards
- Support clients through crises, ensuring timely interventions
- Promote independence, wellbeing, and access to community resources
Requirements
- Qualified Social Worker (Degree in Social Work or equivalent)
- Registered with Social Work England
- Experience working in health, social care, or community settings
- Strong communication, assessment, and problem-solving skills
- Ability to work independently and as part of a team
- A compassionate and patient-centred approach
- Full UK driving licence (preferred but not essential)
