We have a fabulous opportunity for a Care Coordinator to join our award-winning team in Coventry (Top 20 on homecare.co.uk for past 6 years). Esteemed Life Care is a well-established, versatile and family-owned Domiciliary home care, Complex and Supported Living provider that is looking to expand their team as part of the growth achieved.

We are on the lookout for someone who loves caring, and would like to work in a Large, Busy growing office providing quality service to clients, and ensure a strong and caring team is developed and managed.

What attributes we’re looking for?

· Have experience using Access-People Planner/Care Planning Software

Achieved or working towards NVQ Level 5 - Recommended

· Confidence in leading a team of carers ensuring that care delivery is of the highest quality.

· Supportive and caring to the service users and the carers.

· Management skills to organise work, mentor and nurture staff to develop a positive and efficient working environment.

· Highly motivated to go the extra mile.

· Show enthusiasm, and to encourage others in supporting each other.

· Excellent organisational and communication skills.

· People’s person, where your attributes increase confidence within our team and also the clients. Natural at building good relationships with service users and their families.

What will your responsibilities include?

· Identify service user needs and compile care plans and risk assessments.

· Deliver training when needed.

· Carry out reviews of existing service users.

· Manage, Be part of a strong Team and show skills to lead the care team.

· Be able to effectively take enquiries.

· Mentor new staff ensuring that they are fully supported and assessed throughout the induction process.

· Ensure compliance, carry out spot checks and medication audits.

· Be part of the care delivery rota when the need arises.

· Ensure all documentation is completed in line with company procedures and CQC requirements by members of the team.

· Encourage new ideas, maintain staff motivation and morale and drive high standards and development of skills within the team.

· Work closely with the Registered Manager/management team.

· Provide office cover when required.

· Be part of the on-call service. (We pay on Top Enhanced On Call rates)

What We Are Looking for:

Essential:

· A minimum of 3 years of Health and Social Care experience.

· Experience in Domiciliary Care, Complex (CHC/ICB), Supported Living Care, Live In Care

· Exceptional coordinating and organisational skills

· Positive, bubbly personality

· A full UK driving license and access to a vehicle.

Preferred:

· Proficient in using Bookings and Rota software experience.

· Achieved NVQ Level 3 working towards NVQ level 5

· A train-to-trainer qualification.

· Teaching skills would be an advantage.

· Knowledge of medication and administration.

· Strong administrative skills and an eye for detail
