Are you a motivated and compassionate nurse with a genuine interest in inpatient rehabilitation? We’re looking for nurses who are committed to providing patients with the best possible experience and treatment and who are ready to take on the challenge of helping us further develop our rehabilitation services.

Our rehabilitation unit is based at South Bristol Community Hospital. Unit 200 is a 30-bedded adult rehabilitation unit supporting the Pathway 2 acute discharge model.

If you are a Registered Nurse who enjoys working with patients to improve quality of life and promote independence, this is an exciting opportunity to join our team. You will be part of an integrated multidisciplinary team, working collaboratively to deliver individualised nursing and rehabilitation care with the shared goal of achieving the best outcomes for our patients.

Our patients present with a wide range of conditions, including stroke, orthopaedic, frail elderly, post-surgical cases, and long-term neurological conditions. We work closely with them to help achieve the highest level of personal independence prior to discharge.

The Unit Nurse is responsible for the assessment, planning, implementation, and evaluation of care for patients, ensuring the highest standards of patient-focused care. Patient safety, dignity, and respect are at the heart of everything we do.

Key Responsibilities

Provide, monitor, and maintain a high standard of safe and effective nursing care through professional actions and clear verbal and written communication.

With support from senior staff, undertake initial holistic assessments and develop, implement, and evaluate care programmes for patients who are acutely or chronically unwell, often with complex needs associated with frailty.

Carry out a range of routine and non-routine tasks, including:

Medication administration

Clinical observations

Planning, implementing, and evaluating care plans

Assisting patients with daily care needs

Health promotion activities

Venepuncture

Planning and coordinating patient discharges

Work Pattern

You will work across 7 days , including nights, weekends, and bank holidays , on a rostered basis:

Day shift: 07:15 – 19:15

Night shift: 19:45 – 07:45
