We have vacancies across C&M-Sefton, Liverpool, Cheshire West/East and Knowlsey.

As a Senior Individual Assessment and Commissioning Nurse, you will play a vital role in relation to completion of robust care needs assessments to help determine eligibility for NHS funding. For those who meet the criteria for NHS funding, you will work in partnership to develop a personalised care support plan to meet their assessed needs which support safe delivery of a high-quality, person-centred package of care either in their own home or within a care setting.

Working with adults and young people transitioning into adult services, you will:

Lead on assessment and case management
Commission safe, effective, and tailored care packages
Ensure care is delivered in a timely, person-centred, and outcome-focused way
Work collaboratively with patients, families, providers, and multidisciplinary teams
Navigate and apply national policy, guidance, and legislation
Support safe and timely hospital discharges and transitions of care
Contribute to performance targets, quality standards, and service improvement

Working pattern

One of the key benefits of this role is its excellent work-life balance:

Monday to Friday
08:00 am – 16:00 or 09:00 – 17:00
No weekends or bank holidays

Who we’re looking for

We’re seeking an enthusiastic and compassionate nurse who is ready to bring their skills into a rewarding commissioning role.

You will:
Be a registered nurse (Adult, Mental Health, or Learning Disabilities)
Have strong clinical assessment and decision-making skills
Be confident working across complex and evolving care needs
Demonstrate excellent communication and partnership working
Be organised, proactive, and committed to delivering high-quality outcomes
Ability to facilitate MDT meetings , produce robust reports capturing care needs (including utilisation of Microsoft Office for accurate documentation)
