Job Overview
Cyfle Cyf is looking for an enthusiastic, creative and compassionate Youth Practitioner to help launch and develop an exciting new service for young people aged 16–24 across South Gwynedd.
This role will cover Pwllheli, Caernarfon, Bangor, Porthmadog, Blaenau Ffestiniog and surrounding rural communities. The successful candidate will deliver regular Drop-In sessions from five Community Hub locations, provide outreach sessions within rural communities and offer rapid responses when young people are experiencing a crisis.
The service is funded through the Housing Support Grant.
About the role
You will:
- Deliver welcoming, safe and accessible Drop-In sessions from Community Hubs and outreach locations.
- Provide information, advice, brief interventions and practical support.
- Support young people with housing, homelessness, benefits, health, wellbeing, education, training, employment and independent living skills.
- Assess needs, risks, strengths and barriers and agree clear actions with young people.
- Make warm referrals and support young people to engage with other services.
- Develop strong partnerships with local statutory, voluntary, health and community services.
- Help map local services and maintain accurate information about referral pathways and available support.
- Involve young people in designing, testing and improving the new service.
- Contribute to the content, testing, accessibility and promotion of a new app bringing young people’s services together in one accessible place.
- Maintain accurate case records and contribute to service monitoring and reporting.
About you
We are looking for someone who has:
- Practical experience of supporting young people or individuals facing complex needs, disadvantage or crisis.
- Excellent communication, listening, engagement and relationship-building skills.
- A compassionate, non-judgemental and strengths-based approach.
- An understanding of homelessness, safeguarding, risk management, confidentiality and professional boundaries.
- The confidence to work independently across a large geographical area.
- Experience of partnership working, referrals or coordinating support.
- Confidence using digital systems and maintaining accurate records.
- The ability to communicate confidently in Welsh and English, both verbally and in writing.
- A full UK driving licence and access to a vehicle for work.
A relevant Level 3 qualification in youth work, housing, health and social care or a related subject is desirable.
Relevant lived experience that can be used safely and professionally to improve the service is welcomed.
