We are seeking an empathetic and driven Senior Social Worker to join our Short Term Support Service in Morecambe Bay.

We're driving forward innovative practice that will make a difference to our communities and we need new social workers to help us achieve our vision of supporting people to achieve what they want to, living independently in a place they call their home.
Key Responsibilities
- Provide a single point of access to community intermediate care services, including reablement and residential rehab
- Support hospital discharges and work with the wider multi-disciplinary team to prevent hospital admissions.
- Utilize the Discharge to Assess model for assessments away from the hospital environment.
- Conduct holistic, face-to-face Care Act Assessments, reviews, and support planning using a strengths-based approach.

- Registration with Social Work England (SWE) or application to register with SWE.
