The Role:
Join a dedicated service that helps older people, including those living with dementia, regain their independence following a hospital stay. As a Discharge Flow Coordinator, you'll play a key role in ensuring people receive the right support at the right time, working alongside a skilled multidisciplinary team to deliver positive outcomes and smooth transitions of care.
What you’ll do:
- Coordinate individuals' journeys through Short-Term Services.
- Build effective relationships with colleagues, partners, families, and individuals.
- Support timely and well-managed discharges from hospital.
- Lead communication and problem-solving across multidisciplinary teams.
- Help ensure high-quality, person-centred outcomes.
