Full job description
This is an exciting opportunity to join the Ealing/ Hounslow Place-Based Partnership at a pivotal time in our journey towards integrated, community-focused care. As we continue to strengthen collaboration across health, local government and the voluntary sector, we are seeking an experienced and innovative Population Health Management Intelligence Lead to drive the development of intelligence and analytics that supports better outcomes for our residents.
The post is hosted by West London NHS Trust, which currently holds the Ealing and Hounslow Integrator function, but the role will work across the wider Ealing/Hounslow partnership, including NHS providers, primary care, local authority services, voluntary and community sector partners, and the West and North London Integrated Care Board.
This is a unique opportunity to influence how population health intelligence is developed and used across one of London's most ambitious place-based partnerships.
You will work at the forefront of integration, helping shape services that improve outcomes, reduce inequalities and deliver better value for local communities. You will collaborate with a wide range of partners across health, local government and the voluntary sector, making a real difference to the lives of Ealing/ Hounslow residents.
We are looking for someone who combines analytical excellence with creativity, curiosity and a passion for improving population health through data-driven decision making.
As the Population Health Management Intelligence Lead, you will provide strategic leadership for population health intelligence and analytics across the Ealing / Hounslow Place Partnership. You will develop and deliver a comprehensive intelligence function that enables partners to understand population need, address health inequalities, evaluate outcomes and support evidence-based decision making.
Working with senior leaders and stakeholders across organisational boundaries, you will lead the design and delivery of analytical products, dashboards, insights and evaluation frameworks that inform strategic planning, service transformation and operational improvement.
You will be responsible for coordinating complex analytical programmes, translating data into meaningful intelligence and ensuring that high-quality information is available to support the Partnership's priorities. The role will play a critical part in embedding population health management approaches across Ealing/ Hounslow and maximising the use of data to improve outcomes for residents.
West London NHS Trust is one of the most diverse healthcare providers in the UK, delivering a range of mental health and physical healthcare and community services. The Trust runs Broadmoor Hospital, one of three high secure hospitals in the country, with an international reputation.
Our high secure services care for patients from South of England and we provide low and medium secure services across eight London boroughs. The Trust also provides mental and physical healthcare in three London boroughs (Ealing, Hounslow and Hammersmith & Fulham). We employ over 5,000 staff, of whom 59% are BME. Our turnover for 2024-25 is over £500m.
The Trust is rated as ‘Good’ overall by the Care Quality Commission. Forensic services are rated as ‘Outstanding’.
The Trust is an established partner and contributor in the development of the evolving North West London Integrated Care System and the Integrated Care Board. The Trust leads the NW London Children and Adolescent Mental Health provider collaborative.
The Candidate Pack provides an overview of the key tasks and responsibilities of the role, and the person specification outlines the qualifications, skills, experience and knowledge required. Please view as attached
The person specification below is not the full person specification, but outlines the criteria against which your application form will be assessed.
