They care.
At Change Grow Live, our values — be open, be compassionate, be bold — shape everything we do. Every day, our teams help people change the direction of their lives, grow as individuals and live life to its full potential.
We Thrive by recruiting for success, recognising and rewarding effort. We are safe, healthy, agile and flexible in how we work.
We’re looking for a passionate and motivated Hospital In-Reach Recovery Coordinator to join our Coventry Service.
You’ll provide a vital link between the hospital and our community-based drug and alcohol services — offering specialist advice, assessment, and support to people admitted with alcohol or drug-related issues. You’ll work collaboratively with hospital teams to ensure people receive the right care at the right time, supporting smooth discharge pathways and reducing substance-related hospital admissions.
This is an excellent opportunity for someone with experience in substance misuse or health-related services who’s ready to take the next step in their career. You’ll be part of a caring organisation that believes in people — and in your potential to make a lasting difference.
Together as a team, we are learning, developing, growing, and identifying future talent and leaders to deliver our mission.
Location: Coventry
Hours: full time, 37.5 per week
Contract Type: Permanent
Please note: Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, salary and payments are pro rata based on contracted hours.
Responsibilities:
- Build effective communication and referral pathways between the hospital and community addiction services.
- Act as the key liaison for acute referrals, providing timely and professional advice to hospital teams.
- Deliver alcohol and drug assessments using recognised screening tools (e.g. AUDIT), offering brief interventions and onward referral.
- Develop strong working relationships across A&E, Gastroenterology, Hepatology, and other specialist wards.
- Provide advanced assessment, planning and evaluation for a small, complex caseload — ensuring safe transitions from hospital to community care.
- Support hospital staff in identifying and managing patients with substance use issues, offering guidance, awareness sessions and training.
- Work with community recovery teams to ensure assertive and effective follow-up of repeat presentations and discharges.
- Participate in service audits and research to improve practice and outcomes.
- Strong understanding of substance misuse and its impact on health and wellbeing.
- Experience delivering brief interventions or working within hospital, health or substance-use settings.
- Excellent communication and relationship-building skills — able to work confidently with hospital colleagues, patients and partner agencies.
- Proactive and able to work autonomously while maintaining strong team connections.
- Skilled in prioritising workload, managing competing demands and working compassionately in fast-paced environments.
- Knowledge of safeguarding and mental health practice.
- A commitment to continuous learning and improving outcomes for people we support.
Everyone at Change Grow Live experiences a sense of belonging — we are inclusive, we engage and connect at all levels, and we create the conditions for success for people who work and volunteer with us.
- 25 days holiday (plus bank holidays), rising by one day each year up to 30 days.
- A paid Wellness Hour each week, plus access to our Wellness Hub and Employee Assistance Programme.
- Contributory pension scheme.
- A wide range of benefits, including discounts on shopping, cinema, travel and more.
- A friendly, supportive and collaborative team environment.
- Comprehensive training, professional development and career progression opportunities.
- A Refer a Friend scheme to help us grow our community of people who care.
