Job Overview
We are seeking a dedicated and compassionate Care Coordinator to join our healthcare team. The successful candidate will play a vital role in organising and coordinating commplex care services, ensuring seamless communication between service users, families, and healthcare staff. This position offers an opportunity to make a meaningful difference in service users lives while working in a supportive and professional environment. Prior office experience is desirable to facilitate efficient administrative operations and effective client management.

Responsibilities

  • Act as the primary point of contact for service users and their families, providing information and support regarding care plans.
  • Coordinate rostering for care team, treatments, and follow-up schedules with healthcare professionals.
  • Maintain accurate and up-to-date service users records using office management systems.
  • Liaise with Clinical staff, social workers, and other relevant parties to ensure comprehensive care delivery.
  • Assist with administrative tasks such as scheduling, data entry, and document organisation.
  • Monitor serrvice user progress and communicate updates to relevant team members.
  • Staff supersion and monitoring.
  • Ensure compliance with healthcare regulations and organisational policies at all times.
  • Deleiver hands on care in field if required.

Experience

  • Previous care co ordinator experience is preferred, demonstrating strong organisational and administrative skills.
  • Previouse experince working for complex care home care devision.
  • Experience working within a healthcare or social care setting is advantageous but not essential.
  • Excellent communication skills, both written and verbal, with the ability to empathise with service users and their families.
  • Proficiency in using office software programmes such as Microsoft Office Suite or equivalent.
