Job Summary
We are seeking a dedicated and organised Care Coordinator to join our team. The successful candidate will play a vital role in managing client care plans, coordinating services, and ensuring the highest standards of support are maintained. This position offers an opportunity to contribute positively to individuals’ lives through effective care management and excellent organisational skills.

Duties

  • Develop and implement personalised care plans in collaboration with clients and healthcare professionals
  • Coordinate services by liaising with care providers, clients, and their families to ensure seamless delivery of support
  • Monitor and review care plans regularly, making adjustments as necessary to meet changing needs
  • Maintain accurate and up-to-date records of client interactions, care schedules, and service delivery
  • Provide guidance and support to clients to promote independence and well-being
  • Organise appointments, transportation, and other logistical arrangements for clients
  • Respond promptly to client enquiries or concerns, escalating issues when appropriate
  • Collaborate with team members to ensure compliance with organisational policies and regulatory standards.

Requirements

  • Proven experience working within an office environment or administrative role (office experience is desirable)
  • Excellent organisational skills with the ability to prioritise tasks effectively
  • Strong communication skills, both written and verbal, with a compassionate approach towards clients
  • Ability to work independently as well as part of a multidisciplinary team
  • Proficient in the use of office software programmes such as Microsoft Office Suite or equivalent
  • Demonstrated organisational ability to manage multiple priorities in a fast-paced setting
  • A caring attitude with a commitment to delivering high-quality support services
  • Flexibility to adapt to changing circumstances and client needs

Pay: £24,420.00-£26,000.00 per year
