York Medical Group is a large, forward-thinking inner-city practice and Primary Care Network, serving a population of 44,000 patients across 8 sites. Our diverse team includes GPs, ANPs, UCPs, Nurses, Assistant Practitioners, Health Care Assistants, Care Coordinators, Link Workers, and Mental Health Workers. We are currently seeking a motivated, friendly, and experienced Administrator to join our fantastic Complex Care Team.

About the Role

The successful candidate will provide comprehensive administrative support to the Complex Care Team.

The working pattern is 13.5 hours per week working Tuesdays and Fridays. This is essential to the role.

Key responsibilities include:

  • Providing safeguarding administration support when required
  • Providing administration support to our clinicians who carry out weekly care home visits
  • Managing and taking minutes at MDT meetings
  • Updating clinical registers and spreadsheets
  • Running audits on the clinical computer system
  • Liaising with patients, families, social workers, and external agencies
  • Assisting with planning and follow-up of patient care alongside the Home Visiting Team
  • Organising MDT meetings and referrals to external services
  • Responding to Group Emails
  • Incoming/Outgoing phone calls with patients/carers/external agencies

About You

The ideal candidate will:

  • Have excellent organisational and time management skills
  • Be confident prioritising workload within a fast-paced environment
  • Possess strong communication skills with patients and healthcare professionals
  • Have good IT skills, including experience using Excel
  • Have administration experience, ideally within healthcare
  • Have working knowledge of SystmOne (S1) – desirable but not essential, as full training will be provided for the right candidate
