Summary

We are seeking a dedicated Admission & Discharge Co-ordinator for Neuro Services to join our team in the UK. This role is vital in ensuring smooth patient admissions and discharges, contributing to our mission of providing personalized neurological care and support. We are seeking somebody with a passion to be part of the organisations expansion plans as we develop new and existing services.
Responsibilities

  • Coordinate patient admissions and discharges efficiently, ensuring all documentation is accurate and complete.
  • Liaise with multidisciplinary teams including neuro rehab consultants, psychologists, physiotherapists, speech therapists, language therapists, and occupational therapists.
  • Maintain detailed records of patient progress and service delivery.
  • Schedule appointments and manage communication between patients and healthcare professionals.
  • Ensure compliance with healthcare regulations and company policies.
  • Support the development of individualized care plans in collaboration with the team.
  • Assist in administrative tasks related to patient care coordination.
  • Contribute to continuous improvement of service processes.
  • Qualifications
  • Nurse background or clinician background required
  • Proven experience in office administration or healthcare coordination.
  • Excellent organizational and communication skills.
  • Ability to work collaboratively within a multidisciplinary team.
  • Strong attention to detail and accuracy.
  • Proficiency in office software such as MS Office.
  • Knowledge of healthcare regulations is a plus.
  • Prior experience in neurological or rehabilitation services is advantageous.
  • Ability to handle sensitive information with confidentiality.
