Our Team is expanding we are looking for 2 additional Care Coordinators to join our team.

Kelly Park Limited is a passionate and dedicated home care provider located in County Durham that strives to empower individuals to live independently in the comfort of their own homes whilst receiving support for a variety of unique care needs and requirements. We are looking for a Home Care Co-Ordinator to join our team and help us deliver the highest standard of care to our clients.

DUTIES & EXPERIENCE:

As a Home Care Co-Ordinator, you will be responsible for managing a designated area of care schedules, working collaboratively with our team of caregivers & assessors to ensure that they have support and resources they need to deliver the best care possible. Your role will involve building strong relationships with our clients and their families, as well as with healthcare professionals and other stakeholders in the community.

You will be responsible for the rostering in excess of 1000hrs of Domiciliary Home Care per week.

An example of key responsibilities includes:

  • Planning 2 weeks in advance care schedules of our Health Care Assistants around their availability using our resource planning software.
  • Formulating solutions for changes in schedules, increases and taking on new care packages.
  • Dealing with a diverse range of characters and personalities you will ensure the company delivers high quality care in the community which is both timely and reliable.
  • Handling enquiries from a variety of external partners including local authorities, contracts, hospitals and family members.
  • You will also be required to support out of office on call service on a periodic basis, this is paid on top of your salary at the hourly rate.

Previous experience and knowledge of Home Care is ESSENTIAL as the successful applicant will take on leadership responsibilities of their designated team.

Essential Requirements:

  • Hold a full driving licence and have access to a car
  • Strong I.T skills
  • Strong communication and customer service skills
  • Experience of managing a busy work load.
  • Ability to anaylse a situation and take decisive action in a timely manner
  • Previous care experience (+NVQ 3 qualified - Desirable)

Shift Pattern available:

Monday, Tuesday, Wednesday, Thursday, Friday

Working Times: 08:30am - 5:00pm

You will also be required to provide telephone back up to our out of hours emergency support team on a rota system between the hours of 7am-8:30am - 5pm -11pm which will be paid as an enhancement to your salary.

The Co-Ordinator position will include the area, governance, and allocation of work of the villages within the area.
